The questions we get asked most.
If yours is not here, the assistant in the corner can usually answer it, or call 0459 947 067.
Getting started
No. Anyone can contact us directly. A referral is only needed if you want to claim through a Medicare chronic disease management plan (from your GP) or through Workcover and CTP (a certificate from your doctor). NDIS and Support at Home clients do not need a referral.
Usually within a week for home visits and often within a few days in clinic, depending on the discipline and your suburb. Tell us if it is urgent and we will do our best to prioritise.
The clinician talks with you about what is going on and what you want to achieve, does an assessment, and agrees a plan with you. For home visits this includes a look at the spaces you use every day. You receive a plain language summary and we send a report to your funding body if one is needed.
Yes, in every discipline. Tell us your preference when you book.
Yes. Our occupational therapists work with children, most often through the NDIS. Physiotherapy and podiatry for children depend on the need, so please ask.
Funding and fees
Your provider engages us as a third party service and we invoice them directly. Speak to your care manager first so the service is added to your care plan. There is nothing for you to pay beyond any contributions agreed with your provider.
We work with plan managed and self managed participants, and invoice at or below the NDIS price guide rates. Contact us to confirm current registration status for agency managed plans.
No. Treatment is billed to the insurer at the rates set by SIRA. You need an accepted claim, a claim number, and a certificate from your doctor. We handle the treatment requests and paperwork.
Yes, in our clinics. Medicare rebates apply for up to five allied health visits a year under a GP chronic disease management plan. Most private health funds offer rebates for physiotherapy, podiatry, occupational therapy and other allied health under extras cover. Home visits are usually funded through Support at Home, the NDIS or insurers rather than Medicare.
Fees depend on the discipline and appointment length. Call us for a quote. We will always tell you the cost before an appointment is booked.
Home visits
Most of metropolitan Sydney, with the shortest waits around the inner west, south west, north west and Ryde areas. Tell us your suburb and we will confirm.
Yes. We work in residential aged care as well as private homes and retirement villages.
For NDIS and package funded visits, travel is claimed within the rules of that program and you will see it on your statement. For private home visits a travel component is included in the quoted fee.
Yes, and it often helps. We will explain what we are doing and what to keep an eye on between visits.
Reports and referrers
Email info@excelsiorcaregroup.com.au with the client’s name, contact details, funding type, plan or claim number if you have it, and any relevant reports. We confirm receipt and tell you which clinician has been assigned. You can also use the Book page.
Progress reports are usually sent within two weeks of the last session. Comprehensive functional capacity assessments take longer because they involve several visits and detailed writing; allow four to six weeks from the first appointment. Tell us the plan review date and we will work to it.
Yes. Allied health recovery requests, progress reports and return to work recommendations for Workcover and CTP claims are part of standard treatment.
Practical
Nothing special. Comfortable clothing, any walking aids or equipment you use, a list of current medications, and any recent reports or letters from doctors or hospitals.
The clinic is at Shop G05, 1 Monash Road. There is street parking nearby and the clinic is at street level with no steps.
Call or text 0459 947 067 as early as you can. We ask for a day’s notice where possible so the time can go to someone else.
Yes, for some services, particularly follow up sessions in physiotherapy and occupational therapy. Initial assessments are best done in person.
